UO Certificate of Insurance Coverage Requirements

In order to comply with the legal requirements of the University, certain documents must be obtained.  Examples of the documents needed from your company are included below.

Exhibit "A" is from the producer, i.e. usually your local insurance agency.  This document verifies the type of coverage, effective and expiration dates of coverage, and coverage dollar limits. The Certificate Holder section should show the University contact name and address (see bottom of Exhibit "A").  Under "Description of Operations", the verbiage pertaining to additional insured should appear:

"State Of Oregon Acting By And Through The State Board Of Higher Education On Behalf Of The University Of Oregon, Their Officers And Employees Are Named Additional Insured."

Exhibit "B" is the endorsement that must come from the insurer's affording coverage, i.e. the insurance company who actually provides coverage to the producer.  This document must be very specific in naming the additional insured:

"State Of Oregon Acting By And Through The State Board Of Higher Education On Behalf Of The University Of Oregon, Their Officers And Employees Are Named Additional Insured."
