RE: Personal Vehicle Use/Passenger Transport Form and
Vehicle Accident Investigation Form
The University of Oregon has been identified as an agency having a high incident rate of vehicle accidents by the state Risk Management Division. Among strategies to reduce that rate is a University wide process for approval of personal vehicle and/or transportation of non-university passengers. This form needs to be filled out whenever a personal vehicle is used on state business regardless if reimbursement is being requested, or a non-University person is to be a passenger in a vehicle traveling on University business. In addition, a Supervisor Accident Investigation form was developed for investigations of accidents occurring in state or personal vehicles. Make sure to complete both pages of the form.
For personnel who routinely use their personal vehicles the form may be filled out to cover a period of one year if the department supervisor agrees. The request to transport passengers in a state vehicle will be need to be filled out each time.
