Mission

The primary mission of Risk Management is to administer and maintain programs to proactively minimize risks and losses related to UO property. 

  • Assist with risk assessment reviews
  • Coordinate appropriate coverage's from vendors and contractors.
  • Assist with filing of claims with Risk Management Division.

 

Insurance Claim Information

UO must seek recovery for all property loss or damage. Report all claims with in 24 hours (business days) to UO Risk Management.

For claims over $2500 please submit the below information to the BAO Risk Coordinator within 60 days of the occurrence.

For major losses (personal injury or property loss of over $10,000) telephone UO Risk Management (6-3190) IMMEDIATELY. If not available, phone DAS Risk Management at 503-373-7475.

Filing a claim:

1) Complete both pages of the STATE SELF-INSURANCE CLAIM REPORT FORM

2) Obtain 2 estimates or bids. (Cost to buy a new item of same/like kind of damaged or lost item)

3) Obtain photographs.

4) Copy of original sales invoice(s).

5) Obtain a police report from the Department of Public Safety and/or the Police Department

Please submit item (1) as soon as possible.

Forward the other information requested above and the information on the back of the form as soon as possible.

Keep copies of all documents for your files.

BAO Risk Coordinator MUST FILE CLAIMS WITHIN 90 DAYS of occurrence to avoid the claim being denied by State of Oregon Risk Management Division.

Activity Waiver

Risk management is put into place to try and protect the State's assets. Various activities expose us to the possibility of lawsuits. Remember, each event may be unique and the risks need to be assessed. We do not have student insurance. Faculty & staff are covered as long as they are acting in the scope of their duties while on state business. Faculty & staff do not need to complete an Activity Waiver.

The use of waiver/release forms by the state universities has historically been to require the waiver/release whenever taking the class off campus, when holding activities that are inherently risky, i.e. rock climbing, etc., or that are clearly outside the parameters of the regular curriculum. The ideas behind the waiver are to have the students be an "informed participant" when participating in activities that may give rise to more risk than the student's "regular curriculum" and/or to defend liability claims and suits that arise out of a student's actions that are outside the control of the university, but the university is either holding or sponsoring the event.

Activity wavier forms may be needed for:

1) Attending an activity in or out of Oregon.

2) Taking a hired bus, state vehicle, or renting a vehicle for state use. Also when using facilities for overnight use.

Again, weigh the risks, what could happen, who would be at fault? It is the departments responsibility to work toward minimizing risks and losses. If in doubt, contact Risk Management for a risk assessment at 6-3190.
